I believe that Meg calls Cas "Clarence" because the malaikat that earned his wings helping Jimmy Stewart in the movie "It's a Wonderful Life" was named Clarence and after all Cas is an angel.

Have anda ever seen the movie "Its A Wonderful Life" At one point Cas basically says he doesn't understand what she is referencing and she says that he should really watch a movie.In the movie, there is an malaikat names Clarence.
