The attack was expected by what Claire have already said to Jin! But I agree with S that Sayid reaction was really unpredictable!

I think the fact that she's gone a little but CRAZY the hug was so unexpected! I did think she was gonna attack Kate at some point because in Claire's eye's Kate had taken Aaron away from her, so the attack scene wasn't a surprise!
